I can't comment for OS/2 but for linux you have to get the excellent 
PCMCIA package by David Hinds (pcmcia-cs-2.6.3) and install it. 
Theorically any kind of PCMCIA modem is recognized and managed.

Personaly, I bought a I/O Magic 14.4 PCMCIA Data/Fax/Voice modem (Cirrus
Logic chipset). It runs nicely under Linux although the voice part is useless.
It is installed as /dev/cua1 and I can use it with xc, kermit, ..... I did 
not find the time to test the fax part yet....
