Why take it out when booting Win95?? 
just go into the modem in Device Manager under general and disable it in 
the device usage box.  Works for me. 

I personally find it much simpler to press a little blue button on the side and loosen the card up on the way out of Linux, and, in reverse, just press it back into place on the way in.

Your mileage may vary..
