The problem lies in the fact that the PCMCIA card is not re-initialized
after resuming from hibernation.  Ejecting and reinserting after resume
should work, as John Kim mentioned.  Shutting down the PCMCIA slots before
hibernation also works, since the cards are auto-detected after a resume
from hibernation.
